A Clinical Survey of Geriatric Anesthesia



Abstract
With the advancement of socioeconomic and medicare, old aged populations over 60 years were increased. So anesthesiologists are faced in their practice with a lot of geriatric patients. They should be considered a multiplicity of preoperative
physiologic and pathologic states which may affect them during perioperative period.
The authers reviewed anesthetic records of 3392 patients aged over 60 out of 32908 surgical cases, which performed at Chung Ang university medical center from January 1987 to June 1992.
@ES The results were as follows:
@EN 1) Total patients over 60 years of age were 3392 cases, and these were subdivided into 54.9% male and 45.1% female patients. And then the most common age group for geriatric anesthesia was 60~64 years(36.5%).
2) There were 45.3%) in general surgery, 18.8% in urology and 14.4% in orthopedic department among the 3392 cases. And the most frequent site were 25.8% in upper abdominal field and 18.9% in lower abdominal field.
3) The rate of elective and emergency operation were 81.8% and 18.9% The cancer patients were 16.6% and trauma cases were 13.1%. the most frequent emergency operation was done in neurosurgery(40.5%) and eye(29.4%) department.
4) The patient's physical status according to ASA classification were 40.9% of class I and 44.4% of class III. There was abscent in class I over the75 years old cases.
5) The preoperative mean valus of hemoglobin, hematocrit, protein and albumin were within normla limits(12.8¡¾2.5 mg/dl, 38.3¡¾5.7%, 6.5¡¾1.2g, and 3.8¡¾1.0). and arterial blood gas analysis were also within normal limits but PaO2 had a
decreased
tendency according to age.
6) Premedication were discontinued in 40.1% for their physical condition and age. The technique of anesthesia was done under general inhalation anesthesia for 89.0% and regional block for 11.0% The common inhalation anesthetics were used
halothan
for
58.05 and enflurane for 41%. Caudal block was occupied in 40% of regional block.
7) The duration of operation time within 120 minutes was 67.0%.
8) Transfusion was done in 23.5%, and mean amounts of transfusion was 3.1¡¾2.8 pints.
9) The cases of transfer to ICU was 12.8% for their poor conditions. The cases of difficult intubation were experienced in 14 cases(0.4%) and cardiac arrest and fibrillation were occured in 4 cases(0.12%) but postoperative death cases were 2
